Start New Water/Sewer Service Application:

A COMPLETED RENTAL AGREEMENT OR PROOF OF OWNERSHIP IS REQUIRED AT THE TIME OF ESTABLISHING UTILITIES.   A $45.00 (non-refundable) service fee applies to establish service, even if the water is currently "on" at the property.  In addition, a deposit is required for tenants at the time of service and the amount is based on credit. The City of Lakeport will pull a soft credit inquiry for all tenants listed on the rental agreement applying for utilities.  Pulling a soft credit check WILL NOT impact your credit score.   The deposit is refundable when the tenant moves out and is applied to the final bill.  No phone calls will be accepted to establish service.   Forms may be accepted by fax or email but payment must be submitted before the application is processed.   Click HERE to download the form.      DEPOSIT BASE:          GREEN = $75.00          YELLOW = $150.00          RED = $250.00

Stop Water/Sewer Service Form:

A form requesting an account to be closed is required to disconnect services.  Services will continue to be billed until account holder contacts the City to discontinue services.   Click HERE to download the form.  All unpaid balances are required to be paid in full by due date or account is subject to collections or tax rollover.  Any deposit remaining will be applied to the closing bill after the stop date.  No phone calls will be accepted to stop water service.  Forms may be accepted by fax or email.

Leak Adjustment Request:

Review the requirements available to water customers when a leak has caused a high water bill.  If a customer meets the minimum requirements, they may be eligible to receive a one-time leak adjustment as a credit on their next water bill.  The request must be submitted on this form.  Click HERE to download the form.
